低能量脉冲强光对面部敏感性皮肤的临床治疗效果观察

【摘要】 

目的 观察低能量脉冲强光对面部敏感性皮肤的治疗效果及不良反应。

方法 采用低能量脉冲强光治疗30例面部敏感性皮肤患者， 540～950 nm治疗手具，能量密度为 7.0 J/cm2， 脉宽15 ms， 1遍，光斑直接重叠10％，1周治疗1次，4次为1个疗程。所有患者于治疗前及治疗结束后1周进行临床评估，并使用VISIA采集图像进行图像分析。

结果 低能量脉冲强光治疗结束后患者自我评估和医生评估，治疗指数≥50%的患者例数分别为19例（63.3%）和16例（53.3%）。VISIA图像分析显示低能量脉冲强光治疗后，面部皮肤的色素斑点和红斑均有非常显著的改善。

结论 低能量脉冲强光治疗对部分难治性面部敏感性皮肤患者有效，且无不良反应。

【Abstract】

Objective To observe the therapeutic effect and adverse effect of low energy Intense pulsed light for facial sensitive skin.

Methods A total of 30 patients with facial sensitive skin were treated with low energy Intense pulsed light (540-950 nm), with the energy density being 7.0 J/cm2, the pulse width being 15 ms, 1 pass, and the overlap of light spot being 10%. The treatment was given once a week, 4 treatments as a course. All patients were evaluated before and 1 week after treatment, and the images were collected and analyzed by VISIA. 

Results According to the self evaluation of patients there were 19 (63.3%) patients with the index of more than 50% and according to physician evaluation there were 16 (53.3%) patients. The pigment spots and red blood streaks of the facial skin were significantly improved after low energy intense pulsed light treatment as shown by VISIA image analysis. 

Conclusions Low energy intense pulsed light therapy is effective for some refractory facial sensitive patients without adverse effect.

面部敏感性皮肤是临床常见病，其发生和化妆品、激素的不正规使用或玫瑰痤疮有关。临床上表现为面部红斑，同时伴有烧灼、瘙痒、刺痛、紧绷等自觉症状。目前常用的治疗方法包括：选择恰当的医学护肤品，避免日光照射，外用钙调磷酸酶抑制剂，口服抗组胺药物等。但临床实践中有不少患者通过常规治疗仍得不到有效缓解。随着光电技术的发展，近年来有学者开始将光电技术运用到敏感性皮肤的治疗中并取得良好的效果［13］。2016年4月至2016年10月我们对33例难治性面部敏感性皮肤患者予以低能量脉冲强光(intense pulsed light，IPL)进行治疗，并观察其效果。

一、临床资料

(一)研究对象

本组共33例，男1例，女32例,年龄23～60岁，平均(37±10)岁。均诊断为面部敏感性皮肤，病程最短4个月，最长17年。中途有3例女性患者因无法按时治疗、随访而退出，最终30例完成本次研究。
